A. NEW STUDENTS

All applicants must be high school graduates. The following documents are required when seeking the admission:

Form 138 (High School Report Card)

Certificate of Good Moral Character

Four colored pictures (Two 1”x 1” and two 2”x 2” white background)

Birth Certificate

B. TRANSFEREE

The Admission Officer and Registration Coordinator shall interview all applicants for accreditation of subjects. The following documents are required when seeking admission.

Transcript of Records or True Copy of Grades

Transfer Credentials

Certificate of Good Moral Character
